There sure is a lot of parperwork involved in health care today!  Much of it is designed to fully inform patients of their rights and are required by law.  Evans Surgery Center is committed to ensuring their patients are aware of their rights and responsibilities.  Therefore, in an effort to allow you adequate time to review these forms, we are making them available on-line.  By taking time to review them in advance, you can save time on the day of surgery.
